Applicant’s Release, Assumption of Risk, and Agreement to Indemnify *

By participating in the 2025 Annual CLCU Art Fair, I agree to the following terms: Release of Liability I, on behalf of myself, my personal representatives, executors, administrators, heirs, successors, and assigns, release and discharge CLCU, its subcontractors, Savoy Recreation Center, event sponsors, and their parent companies, subsidiaries, affiliates, directors, officers, trustees, officials, members, employees, volunteers, agents, attorneys, successors, and assigns (collectively referred to as “Releases”), from any and all liability for injury, damages, death, property damage, or other losses arising out of or connected in any way with my participation in the 2025 CLCU Art Fair, whether caused by the negligence of the Releases or otherwise. Indemnification I agree to indemnify and hold harmless the Releases from any loss, liability, or damages incurred as a result of my acts or omissions in connection with the 2025 CLCU Art Fair Assumption of Risk I assume full responsibility for the risk of injury, damage, death, property damage, or other losses arising out of or connected in any way with my participation in the 2025 CLCU Art Fair, regardless of whether such losses are caused by the negligence of the Releases or otherwise. Event Cancellation I acknowledge that if the event is canceled due to weather, terrorism, Acts of God, or other circumstances beyond the control of CLCU and Savoy Rec Center, my booth fee may not be refunded. Severability I agree that the provisions in sections 1 through 4 are intended to be as broad and inclusive as permitted by Illinois law. If any part of this agreement is found to be invalid, the remaining provisions will remain in full force and effect. I have read and fully understand this agreement, including the release, assumption of risk, and indemnity terms. By signing, I voluntarily accept and agree to these terms.
